Community Based Homes
Community Based Homes serves individuals with developmental disabilities and
mental health issues who require various levels of residential support. Group Homes provide a range of interventions that focus on training and support in one or more of the following areas: eating and drinking, toileting, personal grooming and healthcare, dressing, communication, interpersonal relationships, mobility, home management and use of leisure time. Capture currently provides community based homes for youth and adults. We have settings for men and women.
Everyday Support
Direct Support Professionals are Capture DCS staff that function as caregivers for Youth and Adults with intellectual and/or developmental disabilities. Direct Support Professional are responsible for supervising, supporting, and guiding habilitation for the individual who is placed in the Community Residential Setting. Our Direct Support Professional runs individualized programming on a daily basis, including Behavior Support Plans and Habilitation Plans, as developed by CDSC’s clinical team members.
